1 / Match the prescription before comparing prices
Write down the exact medicine, strength, dosage form, quantity and days of supply. A quote for a different formulation or quantity is not an equivalent offer. Ask your pharmacist to confirm the comparison.
Keep treatment decisions with your prescriber and pharmacist. Do not delay needed treatment, split doses or substitute a medicine just to match a price. This checklist is a record-keeping tool.
- Record the product specification and quote date.
- Ask for the insurance claim price and an available cash price for the same prescription.
- Confirm stock, pharmacy network eligibility, delivery costs and refill conditions.
| Field | Your record |
|---|---|
| Medicine / strength / form | ________________________ |
| Quantity / days of supply | ________________________ |
| Pharmacy / date / final quote | ________________________ |

2 / Compare the route, not only the checkout number
A lower cash quote can sit outside the benefit claim. Ask the plan whether that purchase counts toward your deductible or out-of-pocket limit. Compare the immediate cost with the consequences for your remaining annual benefit.
Discount and assistance programs can have eligibility and combination restrictions. Ask the pharmacy for the final price and the plan for its own accumulator rules. Do not add discounts together unless the program explicitly permits it.
- Check cash-payment credit with the plan; save its answer.
- Ask whether a clinically appropriate generic is available.
- For a rejection, request the written reason and appeal/exception instructions.
| Route | Price today | Benefit credit confirmed? |
|---|---|---|
| Insurance | ________ | ________ |
| Cash / eligible discount | ________ | ________ |

3 / Preserve the record and choose the next question
Keep receipts, denial notices, benefit rules and dated responses in one folder. Share stories publicly without prescription identifiers or private medical details. Expensive care alone does not identify a rebate, reimbursement spread or violation.
For a hospital bill, request itemized charges and compare them with the explanation of benefits and prior payments. This is a separate workflow from a prescription coverage appeal. Eligible uninsured/self-pay estimate disputes have specific CMS conditions and deadlines.
- Use the plan appeal route for a coverage decision.
- Ask the billing office to explain a specific hospital line item.
- Check current CMS eligibility before using a federal estimate-dispute process.
Please confirm the exact medicine, strength, form, quantity and days of supply priced. What is my insurance price and your available cash price? If the claim was rejected, please provide the reason and the written exception or appeal route. I will confirm deductible credit with my plan.
